• Be a permanent or seasonal Florida resident
• Be diagnosed with a qualifying medical condition
• Be entered into the Medical Marijuana Use Registry by a qualified physician
• Obtain a Medical Marijuana Use Registry Identification Card

To be eligible for Medical Marijuana Certification, you must be diagnosed with a qualifying medical condition, or a comparable medical condition of the same kind or class.  Certain conditions treated by a physiatrist will qualify.  Dr. Ettedgui will be able to make that determination and recommendation, and instruct patients in the registration process.

For new patients requesting Medical Marijuana Certification, prior medical records will be required to document your qualifying medical condition.  You may bring copies of the records to your first appointment.

There are various types of Medical Marijuana, as well as Low-THC.  There are also various routes of administration available, including:

• Inhalation
• Sublingual 
• Oral & Edibles
• Topical
• Suppository
• Smokable Cannabis

Dr. Ettedgui is trained in recommending the proper route and strains to obtain the best result to treat your condition.

Chronic nonmalignant pain caused by a qualifying medical condition or that originates from a qualifying medical condition and persists beyond the usual course of that qualifying medical condition can be considered a qualifying condition for Medical Cannabis.

Medical Marjuana Certification fees are paid in CASH only.  No credit cards, checks or debit cards.

The initial certification visit cost is $180.

Subesequent recertification visit cost is $125.

There will be additional licensing fees, paid to the State of Florida, at the time of your registration, and annually for renewal.
